Records Requests

You may request copies of EarthSong records for yourself, or to be transferred to the clinic of your choice using this form: Release of Records Request

You may return this to me via email (as noted on the form), postal mail, or fax. I will respond within one week.

Records Request Fees: All records requests will be processed once weekly, and may incur a $25 fee (depending on the size of your chart, and method selected for transfer of records).
